一名地方医保局局长提出应实行"精神病院五查" 有医保专家对第一财经表示,与普通医疗机构相比,医保在对精神病院的监管中存在诊疗行为是否合规定性困难,付费机制特殊、处置风险高、地方顾虑较 多等难点。 "当前医保监管中面临最核心的问题是医保部门难以独立判断精神病院的诊疗行为是否符合诊疗规范,如将正常人虚构精神疾病收治入院,戒酒者入院等, 这需要卫生健康部门介入专业认定。"这位医保专家表示。 另一个地方医保局局长对记者表示,精神病患者需长期住院,精神病院一般不实施DRG/DIP(按病组/按病种分值付费),而实施按床日付费,一般以30日、60 日、90日为界逐步递减给予不同的支付标准。由于住院日与经济利益直接挂钩,诱导机构延长住院或虚假收治。过去有些地方有一年不超过多少日的限制, 但近几年很多地方都取消了住院时长限制,不少病人事实上常年住院。而且因为封闭管理,一些康复性心理性治疗又无药品耗材应用,医保很难监管到位。 与一般医疗机构相比,医保机构查出精神病院违规违法行为之后处置起来的难度也比较大。按照医保基金监管的相关法律法规,医保部门对骗保的定点医疗 机构可采取解除医保协议的措施,但地方政府会担心解除协议可能造成院内真 ...
